Our process begins with an initial assessment for each resident. The resident’s needs and Hands at Home ability to meet those needs are considered prior to placement. We make every effort to house a compatible mix of satisfied residents. After the initial assessment, we then move to the next step, which is developing an individualized plan of care. The program manager updates the plan of care regularly with resident’s updates and progress. Our goal is to meet the needs of each resident and to adapt a plan of action as needed.
OUR LEADERSHIP
Clarindria (Claire) Addison
After earning a Bachelors of Science degree in Educational Policy and Community Studies Cum Laude, from the University of Milwaukee, Wisconsin. and a Master’s in Community Mental Health, with a minor in Substance Abuse Counseling from Southern New Hampshire University, Clarindria entered the industry as a Licensed Professional Counselor with the State of Wisconsin to follow her passion. The foundation of her work comes from a systems theory. Her ability to view issues within context to incorporate the use of a multi-modal therapy of: cognitive behavioral, narrative, strategic, systemic, strength based and solution-focused theories to gain perspective and objectivity. Clarindria's passion motivates others to reach their greatest potential. Her joy stems from seeing the growth of the lives she touches consistently, while her happiness comes from knowing that she is intentional about saving lives every day.
Addison has over twenty years of entrepreneurship experience, which has yielded multiple successful business ventures including: a coaching/consulting business, trucking company and a Community Based Residential Facility(CBRF). Additionally, she has worked as both a juvenile/adult probation officer and Supervisor for the State of Wisconsin. She has also worked as a Wraparound Care Coordinator and Supervisor for a multimillion dollar community based service organization.
Clarindria's talents extend to the classroom, where she also serves as a facilitator, and program creator who loves to empower people. In her upcoming book being released in 2021, Clarindria provides the blueprint to business success that helped her overcome obstacles and grow in the midst of adversity. Most recently she has partnered with American Family Insurance and Major League Baseball's team, the Milwaukee Brewers to provide a series of courses to contribute to their outreach efforts.
Clarindria is a mother of three successful adult children and currently reside in Wisconsin.

This facility is a well-maintained single family home, located in a quiet residential community. We are dedicated to helping individuals maximize their full potential in a compassionate, caring, and competent environment. Hands at Home offers a continuum of care and services enabling us to meet the needs of individuals with a wide range of disabilities.
